Application for Practice Staff

This application is for practice staff wishing to join our ‘Practice Plus’ or ‘Practice Plus Extra’ scheme. Eligibility for the schemes is only open to practices whereby 50% of the GP staff working at the surgery are members of MDS.

MDS is able to offer membership to only practice nurses, phlebotomists, Healthcare Assistants (HCAs) and Practice Manager.
